CHSP Service Coordinator - 12 month contract
Reporting to the Program Manager, this is a short term contract for 12 months as a Coordinator for the Commonwealth Home Support Program (CHSP) within the BCS My Links team.
Demonstrating a person centred approach in all aspects of your work you will be responsible for the evaluation of new referrals received from My Aged Care. In consultation with the participants and /or their carers/advocate, care plans are developed that reflect an individuals needs to enable the participant to live the life they choose to live.
Care plans for new and existing participants are to be reviewed on a regular basis to ensure services being delivered reflect the current needs of our participants.
You will be responsible for supervising support workers to ensure that services identified in a participants care plan are delivered efficiently, safely and effectively.
Monthly reporting requirements include output monitoring and reporting as well as financial variance reporting.

This is a 12 month fixed-term, full time position with an annual salary of between $74,238.32 and $78,585.52.
